Preparing for a Sports Physical
Sports physicals are important appointments for your child. Sports physicals are required each year and our comprehensive exam also includes the yearly check-up.
We will complete the sports and school forms at the time of the visit. Most insurance plans will cover the exam, but it’s always best to review your benefit package.
Any immunizations your child needs will be discussed and administered. We review the general health of your child including blood pressure, growth, BMI (Body Mass Index), nutrition, exercise, mental and emotional well-being. Any risk factors for sports participation are evaluated and addressed. Concerns regarding behavior, school or social performance are also addressed.
These are some of the reasons why Pediatric Care of Kentucky recommends sports physicals/checkups at our office every year.
Consent for Appointments
If a parent or legal guardian is unable to accompany their child (younger than 18 years of age) to our office, please make sure that you provide us with a written authorization giving us permission to treat the child in your absence. The person accompanying the child will need to pay any co-pays etc. on behalf of the parent or legal guardian.
