You may schedule well child appointments with the doctor of your choice. We do recommend that you try to meet all the doctors in the group at least once.
During a well-child (also called a preventive or check-up) visit our doctors provide services and vaccines based on a care plan called Bright Futures, which was developed by the American Academy of Pediatrics (AAP). These well-child visits focus on growth and development, early detection of problems through screening examinations, laboratory tests and recommended vaccines.
Following the standards of care established by the Academy of Pediatrics, you will be asked to fill out a MCHAT questionnaire to screen for autism at the 18 months and 2 year old visits. Download it here.
Your child will be asked to complete a mental health screening tool beginning at the 12 year physical and then every year or when deemed necessary. This is a standardized screening tool and is to be completed by your child without input from parent. You may download it here.
You may also download school-related forms for sports and physicals that will need to be completed BEFORE your visit from our Forms page. Forms not completed at your visit could result in a clerical fee if they are requested at a later date.

Consent for Appointments
If a parent or legal guardian is unable to accompany their child (younger than 18 years of age) to our office, please make sure that you provide us with a written authorization giving us permission to treat the child in your absence. The person accompanying the child will need to pay any co-pays etc. on behalf of the parent or legal guardian. The Consent Form is available from the Forms page.
